Riser Hoyt Horizon Limb Pocket adjustment

I have a HOYT horizon 25" Riser.

When we came to tune the bow properly it was quickly evident that the string wasn't running central. This again was easily identified to the fact that the upper limb pocket adjustment was all the way over to one side.

We adjusted the limb pocket to the centre, then checked it on a bow vice and adjusted until it was spot on.

Now a few weeks later, I thought I'd just give it a quick visual check and that same limb pocket is back over to the side again.

So it doesn't seem to be holding.

The limb pocket has a bolt, then either side there is a grub screw. There is then a second grub screw which I presume acts as a locking grub screw? This is the same on the lower limb pocket as well. There is however one of the locking grub screws missing on the upper one where I am getting the movement. However it isn't on the side it has moved towards it is on the other side!

So my first question - is there suppossed to be 2 grub screws in each hole?

Second question - why is it moving? Is the thread likely to be stripped?

Third question - what is the best way to solve this?

Fourth question - does anyone know the grub screw size and thread in order to get a replacement?
